Residency Forms

  • Student Declaration of Citizenship Form
  • – Print and complete this form if you have already applied for admissions and are going to submit your identification or documents in person. This form will not be processed via email or fax. If you would like to submit your ID or other Lawful Presence documents electronically, please visit: www.my.maricopa.edu/id.

  • Dependent Domicile Affidavit
  • – Print and complete this form if you have previously attended SMCC and/or were billed as an out of state student and are dependent of a parent or spouse who has residency within the state of Arizona, and would like to petition for Maricopa County residency. This form and supporting documents must be submitted in person and prior to the start date of the semester you are petitioning for resident tuition.

  • Independent Domicile Affidavit
  • – Print and complete this form if you have previously attended SMCC and/or were billed as an out of state student and would like to petition for Maricopa County residency. This form and supporting documents must be submitted in person and prior to the start date of the semester you are petitioning for resident tuition.

  • Western Undergraduate Exchange (WUE) Form
  • – Print and complete this form if interested in enrolling under the Western Undergraduate Exchange (WUE) Program. Students participating in WUE must submit their petition prior to the start of the first semester they enroll in courses. Students maintain their residency with their home state and pay a discounted out-of-state rate. Students who are residents of Alaska, Arizona, California, Colorado, Guam, Hawai’i, Idaho, Montana, Nevada, New Mexico, North Dakota, Oregon, South Dakota, CNMI and Guam, Utah, Washington, and Wyoming are eligible to participate in the program. Once WUE is granted, a student not eligible to petition for in-state tuition.
