Enrollment Process and Forms for VA Education Certification

The following forms can be on the VA benefits Website:

  • Students must fill out an Application for VA Educational Benefits also known as VONAPP (Form 22-1990).
  • Request For Benefits (RFB) form: This form needs to be completed and turned into the Veterans Services Office after you enroll EVERY SEMESTER.
  • Change of Place or Training: This form needs to be completed if you are changing PARENT schools for VA Benefits, or if you are changing your declared PROGRAM of STUDY with Veterans Services.
  • If you are enrolled or plan on enrolling at a Guest School and using your VA education benefits you will have to fill out a Parent School Letter. The form will go through approval at Parent/Home School first then be forwarded to the Guest school for certification.

Please keep in mind that students have to see an academic advisor every semester. Your advisor will provide you with an Advisement Report and an Education Plan. Please bring these forms to Veterans Services. Everyone receiving education benefits MUST complete a VA Deferment (promissory note) at our office every semester after registering for classes. This is important so that your classes will not be dropped on the tuition and fee payment due dates.
