If you are under the age of 24 and a dependent of one or more parent for federal income tax purposes you will need to complete and submit the Dependent Student Domicile Affidavit.  The purpose of this domicile affidavit is to provide information in regard to the domicile of the student.Domicile means a person’s true, fixed, and permanent home and place of habitation in Arizona for the 12 months preceding the official start of the term in which you intend to enroll. This form includes a list of documents (depending upon date of the document) that would be considered relevant in determining a student's residency status

There are certain exceptions to the 12 month rule. You may be eligible for in-state status if one of the following applies before the first official day of the semester.

Your parent/guardian will receive an email inviting them to review and electronically sign this form after the student has completed their portion of the form.

FERPA (Family Education Rights and Privacy Act of 1974).  The student gives permission to release Maricopa Community College student information to their parents/guardians in accordance with Federal Law. Also includes an enrollment proxy.

This form is specifically designed for use by Arizona Agribusiness and Equine Center (AAEC) students in partnership with SMCC. By signing and submitting this form, students designate the AAEC Acting Principal or Executive Assistant to perform enrollment actions (i.e. add, drop, swap, and/or withdraw from classes) on their behalf.

This educational consortium agreement allows out-of-state students to take courses offered by MCCCD and pay tuition of 150% of in-state tuition in accordance with state statute.  Students who reside in western states (Alaska, California, Colorado, Guam, Hawaii, Idaho, Montana, Nevada, New Mexico, North Dakota, Oregon, South Dakota, Utah, Washington, and Wyoming), the Commonwealth of the Northern Marianas Islands (CNIMI) and who meet the eligibility requirements may apply for the WUE tuition rate.
