CARE (Early Alert) Reporting

  1. Academic (need tutoring or additional resources to help with their classwork, etc.)
  2. Attendance related (not showing up or logging in for class on a regular basis, etc.)
  3. Non-academic issues (reports of homelessness, COVID-related issues, self-harm, suicidal thoughts, nutrition assistance, etc.)


Student Conduct Reporting (Public Incident Report)         
NOTE: If you are unsure of which form to choose, please use this form.

  • This form can be used to report general student misconduct or student code violations (e.g., cheating, forgery, plagiarism, violence, threats).


Discrimination & Sexual Harassment (Title IX) Reporting

  • This form is used to report discrimination or harassment based on protected categories as listed in the MCCCD administrative regulations (race, color, religion, sex, sexual orientation, gender identity, national origin, citizenship status, age, disability, pregnancy, or veteran status).
